about our clinic and how to become a patient:

Shifa was founded in 2015 in an effort to serve those in need. Anyone can become a patient, and all are welcome. No appointments are ever needed, and walk-ins are welcome. 

Services offered: free health screenings, written prescriptions, consultations, over the counter medications, and discounted lab services. Give us a call if you have questions or medical concerns about what we offer. If we are unable to offer a service, we may be able to direct you to someone who can. 

 

Clinic is held every 2nd and 4th Saturday of the month.

Financial Requirements: 

The Shifa Free Health Clinic was founded to meet the needs of the uninsured and underserved members of our community. The volunteers are here to help those truly in need. To make sure that all patients meet these requirements, all patients must be UNINSURED and be at 250% of the federal poverty line. Patients do not need to be local residents and NO evidence of documentation is required. We do need a form of valid ID to confirm who we are treating.

Acceptable forms of ID include:

  • Driver’s license

  • DMV ID card

  • Passport

  • Valid Green Card

 

Acceptable forms of financial-need documentation include: 

  • If patient is employed, then we need a copy of their paycheck stub (last 4 weeks) or letter from employer.

  • If patient is self employed, then we need copy of their most recent tax return.

  • If patient is unemployed, then we need letter of support from whomever is supporting them. For example, information about the person who is feeding and giving shelter them. We need that information from the person him/herself. See attached letter of support form.

  • Child support, Alimony, Food Stamps, SSI
